Agency Initial Licensing

To be recognized and certified as an EMS Agency in Wyoming, the following requirements must be met:

1. Have medical direction through a Wyoming Physician licensed to practice in Wyoming.
2. Current Protocols reviewed within the year the application is submitted.
3. For any Agency wishing to be identified as a Transport Agency, it must have a copy of a certificate of insurance, issued by an insurance carrier licensed to do business in Wyoming, whose certificate shows that each ambulance owned or
   operated by the ambulance service company is covered by insurance, providing for the payment of benefits and damages in at least the following amounts:
   1. (A) Liability coverage in the amount of one million dollars ($1,000,000.00) for each individual claim and two million dollars ($2,000,000.00) for personal
      injury or death claims arising out of any one (1) motor vehicle accident, or the limits allowed to participants of the state’s Local Government Liability Pool;
   2. (B) Liability coverage in the amount of one hundred thousand dollars ($100,000.00) for property damage claims arising out of any one (1) transaction or
      occurrence, or the limits allowed to participants of the state’s Local Government Liability Pool; and
   3. (C) Liability coverage in the amount of two million dollars ($2,000,000.00) for personal injury, death, or other claims arising out of any one (1) transaction or
      occurrence, or the limits allowed to participants of the state’s Local Government Liability Pool.

EMS Ambulance Services are broken into two categories:

1. Transport Agency – Has an Ambulance and the capability to transport a patient.
2. Non-Transport Agency – Only Personnel and support EMS operations/patient care.

To initiate the process of becoming a Certified and Recognized Agency in the state of Wyoming, please follow the steps below:

1. The Agency point of contact needs to email EMS-Licensing@wyo.gov the following information: 
   1. Primary Point of Contact’s information to include: 
      1. Name, first and last.
   2. Medical Director information to include: 
      1. Name, first and last. 
   3. Agency information to include: 
      1. Agency Name.
2. Both individuals will need to have personal OWLS accounts created before the application process can begin. 
3. Once the above information has been received, the Office of EMS will create the Agency shell in OWLS.
4. The Primary Point of Contact will receive notification that an Initial Agency application has been associated with their OWLS account.
5. The Medical Director will receive notification that an Initial Medical Director Affirmation form has been associated with their OWLS account.
6. The Primary POC completes the Agency Application and submits it with all supporting documentation as required through OWLS.
7. The Medical Director completes the Medical Director Affirmation form and submits it through OWLS.
8. If you are a Transporting Agency, the $20.00 Licensing fee must be submitted to the division by certified mail, return receipt requested, to the Office of EMS before the application can be completed. (Does not apply to Industrial Transport Ambulance Services)
9. The Office of EMS may inspect the ambulance or designate a designee to inspect the ambulance.
10. Agencies will then complete the Wyoming Ambulance Trip Reporting System (WATRS) orientation.
11. The Agency will then receive its Certificate of Operation via OWLS correspondence and may begin operations.

Agency Renewal Process

Agency renewals open on October 2 of each year. Renewal applications must be submitted no later than December 1 each year to allow the Office of EMS time to process each application. The renewal closeout date is December 31 of each year.

Agencies will have access to the Agency Renewal application through their OWLS portal starting October 2. The agency will need the following information in order to complete an Agency renewal application.

1. Up-to-date protocols that have been signed and reviewed within the last two years by the Medical Director.
2. Medical Director Affirmation completed through the application process.
3. For any Agency wishing to be identified as a Transport Agency, it must have a copy of a certificate of insurance, issued by an insurance carrier licensed to do business in Wyoming, whose certificate shows that each ambulance owned or
   operated by the ambulance service company is covered by insurance, providing for the payment of benefits and damages in at least the following amounts:

4. Each Ambulance on the Agency roster will need a self-inspection completed through OWLS.
5. The Agency will need to complete an Agency Self-inspection through OWLS.
6. If you are a Transporting Agency, the $20.00 Licensing fee must be submitted to the division by certified mail, return receipt requested, to the Office of EMS before the application can be completed. (Does not apply to Industrial Transport Ambulance Services).

Renewal Applications will be processed as they are received.
