WDH creates, receives, maintains and transmits a large amount of information, including personally identifiable information and protected health information. The OPSC supports WDH programs and facilities in making sure that information is only used and disclosed as allowed by law and making sure that information is adequately protected.
How do I request a copy of my health records from WDH programs?
Clients may request a copy of their health records maintained by the Wyoming Department of Health by completing the WDH Authorization to Release Health Records. For Spanish-speaking clients, complete the Autorizacion para Acceso al Historial Médico Departmento de Salud de Wyoming.

How do I request an amendment to my health information?
Clients have a right to request an amendment to their health information maintained by the WDH if it is believed to be incorrect or incomplete by submitting an Amendment Request form.

How do I request an accounting of disclosures?
An accounting of disclosures is a list or statement of all the times WDH has given a client’s protected health information to a program, entity or person.
Clients have a right to request and receive an accounting of protected health information disclosures made by WDH for a period of time, not to exceed six years, by completing an Accounting of Disclosures Request form.

How do I request restrictions on WDH's use and disclosure of my protected health information?
Clients may request that their information is not used by WDH or that their information is not given to certain people who would otherwise have access to it by completing a Use and Disclosures Restriction form.
