Children’s Special Health Program

• Call your local Public Health Nursing or Indian Health Service office to set up an appointment to complete an application, including program Rights and Responsibilities.  Click here to view our Rights and Responsibilities form.
• Bring your medical and financial records with you to this appointment.
• After your application is processed, you will receive a letter notifying you of eligibility or denial for the program.

Care coordination services provided through the CSH program include:

• Working with the client and their family to identify needs, concerns, and priorities
• Supporting families in following the client’s plan of care (for example, tracking and providing appointment reminders)
• Locating, accessing, and connecting families to needed community services and resources
• Ensuring services are coordinated among interdisciplinary team members and across programs and agencies
• Ensuring families have access to health care coverage (for example, helping families sign up for Medicaid, Kid Care CHIP [Children’s Health Insurance Program], or insurance through the Marketplace)
• Investigating billing problems
• Providing support for transition to adult health care services
• Evaluating the effectiveness of service delivery in meeting client and family needs

To qualify for gap-filling financial assistance*, the applicant must:

• Be a Wyoming resident
• Be a U.S. citizen or qualified alien

• Meet medical eligibility – for more information, click here to view current medical conditions eligibility criteria.
• Families with health insurance, Medicaid or Kid Care CHIP may be eligible
• Families may be required to apply for Medicaid/Kid Care CHIP

*Maximum financial benefits:

• Children’s Special Health (CSH) – $40,000 annual limit per child for covered services/provider(s)
• Maternal High Risk (MHR) – $5,000 per case, including travel assistance and $5,500 towards air ambulance transport
• Newborn Intensive Care (NBIC) – $40,000 per case, including air ambulance transport and travel assistance

CSH Services are available to children and youth (ages 0-18) with special health care needs who have, or are at increased risk for, a chronic physical, developmental, behavioral, or emotional condition, and who require health and related services of a type or amount beyond that required by children generally.  Examples of covered medical conditions include:

• heart conditions
• genetic conditions
• cleft lip/palate
• cerebral palsy
• Diabetes type 1
• seizures

MHR Services are available to high-risk pregnant women who receive care from an out-of-state Level III provider, such as Denver, Salt Lake City, and Billings.

NBIC Services are available to high-risk infants who are delivered at, or transferred to, an out-of-state Level III nursery, such as Denver, Salt Lake City, and Billings.
