Information for Enrolled Providers Regarding the Changes to the WyVIP Program
Facilities that are no longer eligible to participate in the WyVIP Program have two options:
- Remain in the VFC Program and meet all VFC enrollment requirements.
- Have available privately purchased vaccine to be administered to patients not eligible to receive VFC-supplied vaccine
- Privately purchased vaccines will need to be entered into the WyIR inventory for review and verification by Immunization Unit staff by December 31, 2020 to remain eligible to order and receive VFC vaccine.
- Complete the VFC Provider Enrollment Agreement by December 15, 2020 and send it to wdh.pvpreporting@wyo.gov.
- All doses of WyVIP-supplied vaccine as of January 1, 2021 in inventory will be considered VFC doses and only VFC-eligible patients will be eligible to receive these vaccines.
- Have available privately purchased vaccine to be administered to patients not eligible to receive VFC-supplied vaccine
- Withdraw from the VFC Program.
- Providers wishing to withdraw from the VFC Program will need to complete the Withdrawal form by December 1, 2020 and submit it to wdh.pvpreporting@wyo.gov.
- Providers will be able to administer their VFC and WyVIP inventory to eligible patients through December 31, however, for planning purposes, we would like the Withdrawal Form completed by December 1st.
- The Immunization Unit will work with provider offices that decide to withdraw from the VFC Program to safely transfer all publicly supplied vaccines from those facilities to a VFC-enrolled provider in your area by Friday, January 8, 2021.
- Providers wishing to withdraw from the VFC Program will need to complete the Withdrawal form by December 1, 2020 and submit it to wdh.pvpreporting@wyo.gov.

Program Enrollment
Are You Ready To Join the Team? All Wyoming immunization providers are welcome to participate in the Vaccines for Children (VFC) Program. As of January 1, 2021, only Federally Qualified Health Centers (FQHCs), Rural Health Clinics, and Public Health offices will be eligible to participate in the Wyoming Vaccinates Important People (WyVIP) Program.

Providers needing to complete the Vaccines for Children Program provider enrollment Agreement to remain enrolled in the Public Vaccine Program, please complete the Vaccines For Children Program Provider Enrollment Agreement form and email it to wdh.pvpreporting@wyo.gov.

Program Withdrawal
Providers that are currently enrolled in a Public Vaccine Program who no longer wish to participate must complete the following process and form.
Providers are financially responsible for publicly supplied vaccines until they are safely transferred to another provider under the approval of the Immunization Program.
