About the PATH grant
SAMHSA’s Projects for Assistance in Transition from Homelessness (PATH) funds services for people with serious mental illness (SMI) experiencing homelessness.
Wyoming contracts with four community providers to assist people who are experiencing homelessness and have a recent diagnosis of SMI, SED, and/or a co-occurring disorder. These agencies can provide coordination services and other resources:
Sweetwater Family Resource Center
Recover Wyoming (Cheyenne)
Community Action Partnership of Natrona County (Casper)
The Wyoming Department of Health, Behavioral Health Division is no longer accepting applications for the 2026 PATH Grant year.
Below are links to last year’s application and guidelines for reference. Application documents for the 2027 PATH Grant year will be posted between January and February of 2026.. All applications and questions should be directed to bhd.grants@wyo.gov.
Intent of the Program: PATH’s goal is to assist individuals who are at imminent risk or are experiencing homelessness and have a serious mental illness or co-occurring substance use disorder(s). The services provided link the individuals in need to mainstream and other supportive services to help secure safe and stable housing, improve health, and overall encourage a self-directed, purposeful life. These funds are not intended to be solely used for housing support (i.e,. rent, safety deposit, etc).
Who can apply: Only Wyoming non-profit organizations, local or county governments, and tribal entities may apply. Applicants must provide any updates to the entity’s Unique Entity Identifier (UEI) number and Certificate of Good Standing from the Wyoming Secretary of State, or any updated letter from the County stating governmental status is required.
