What is the High Fidelity Wraparound Program?
High Fidelity Wraparound (HFWA) is a program for youth—ages 4-20—with complex behavioral health conditions.
Program services are provided through a partnership between Magellan of Wyoming—which manages the children’s mental health waiver—and the Wyoming Department of Health, Division of Healthcare Financing (Medicaid).
Often, youth experiencing complex behavioral health conditions are at risk for out-of-home placement.
Through the HFWA program, youth and their families build a system of support—following a team-based, individualized process that prioritizes family voice and choice—and the combined support of that system addresses behavioral needs while allowing the youth to stay in their home, school, and community.
Primary care and community mental health providers are key players on the High Fidelity Wraparound team.
The HFWA program is an opportunity to support youth and their families in addition to the care you already provide.
You make the referral; the Wyoming HFWA team will take it from there.
Part of the referral process includes completing a Level of Care assessment, a simple, streamlined, one-page document that’s designed to reduce complexity, support timely access to services, and create a more user-friendly experience for families and providers alike.
While your participation in a youth’s support team isn’t required, your involvement is preferred for the youth’s best outcomes.
