What is a Psychiatric Residential Treatment Facility (PRTF)?
Psychiatric Residential Treatment Facilities (PRTF) are 24-hour, supervised, inpatient level of care for children and adolescents up to age 21. A PRTF is not a hospital. The PRTF facility must be accredited. There are approximately 15 PRTFs contracted with Wyoming Medicaid.
Members in PRTFs have long-term mental health illnesses. They are not likely to respond to short-term interventions. They have also not responded to community-based intervention(s).
Psychiatric Residential Treatment Facility Admissions
A PRTF may be right for a child or adolescent if he/she has a psychiatric condition that cannot be treated at home or in a less restrictive setting. Symptoms are typically severe.
PRTF services should not be used for a client with sudden and emergent psychiatric behaviors. If a client is experiencing a medical emergency, he/she should visit the emergency room.

Psychiatric Residential Treatment Facility Incidents
The following list of incidents involving Wyoming Medicaid members that occur at a PRTF must be reported to the State of Wyoming Department of Health within one business day of the incident:
- Physical or sexual assault
- Injury or illness that requires medical attention, including caused by restraint
- Medication error requiring medical attention
- Involvement of law enforcement, outside of WY DFS/probation
- Harm to self-requiring medical attention
- Fire or other disaster affecting client’s living or treatment situation
- Elopement or abduction
- Illegal substance use while on facility grounds or on a facility supervised trip
- Pregnancy if not known at time of admission
- Discharge to the wrong facility
- Death
What is an Onsite Compliance Review (OSCR)?
The purpose of an onsite compliance review (OSCR) is to make sure that the PRTF complies with all State and Federal requirements for mental health treatment. It also makes sure Wyoming Medicaid members get quality treatment.
The goals of the OSCR are:
- Review the programs and services offered by the PRTF. Health professionals will visit the facility in person. They may also review records off site.
- To provide clear feedback to the PRTF about findings in order for services to be improved.
The frequency of OSCRs depends on the status of the facility at the last review. Typically, the higher the facility’s rating, the longer the time between reviews.
After the OSCR has been completed, the PRTF will be provided with their status.
Psychiatric Residential Treatment Facility Status Categories
At the end of the OSCR the PRTF will be provided with their Status Category. The Status Categories are:
Commendation: Program and services consistently exceed standards. No problems were cited by the review team. The next OSCR will be scheduled within the next three (3) years.
Approved: Program and services consistently meet standards the majority of the time. No significant health and/or safety concerns were cited by the review team. The next OSCR will be scheduled in one (1) to two (2) years. A corrective action plan (CAP) may be requested at the State’s discretion for findings cited.
Review: Overall program and services are of acceptable quality with one (1) or more specific areas of health and/or safety risk or other substandard quality directly impacting the quality of effectiveness of services delivered. A CAP must be submitted to all address findings cited. The next OSCR will be scheduled within the next six to twelve months after the implementation of an approved CAP.
*Please note: If a PRTF has a “pending” or “hold” status, this means that the facility has not had any Medicaid members placed there in quite some time – essentially, a future review is pending or on hold until there are members placed there.
