The Patient Centered Medical Home (PCMH) is a care model for achieving primary care excellence so that care is received in the manner that best fits a patient’s medical needs. Providers that participate in the PCMH program have made a commitment to continuous quality improvement and a patient-centered approach to care. It has become a widely accepted model for how primary care should be organized and delivered throughout the health care system, and encourages providers and care teams to coordinate on their patient’s care from the simplest to the most complex conditions.Clinics

What can you expect from your Patient Centered Medical Home?
Same Day Appointments – Have a health concern? Your PCMH clinic must reserve appointment slots each day for their clients!
Extended Hours – Your PCMH understands that not everyone is able to make it into the clinic from 8 a.m. to 5 p.m. That’s why PCMH clinics offer extended hours – to be there when you need them!
Comprehensive Care & Care Coordination – Your PCMH clinic is accountable for meeting the large majority of your physical and mental health care needs, including prevention and wellness, acute care, and chronic care. Providing comprehensive care requires a team of care providers. This team might include physicians, advanced practice nurses, physician assistants, nurses, pharmacists, nutritionists, social workers, educators, and care coordinators. Although some PCMH clinics may bring together large and diverse teams of care providers to meet the needs of their patients, many others, including smaller practices, will build virtual teams linking themselves and their patients to providers and services in their communities.
Patient-Centered Care – Your PCMH provides health care that is relationship-based with an orientation toward the whole person. Partnering with you and your family requires understanding and respecting your unique needs, culture, values, and preferences. Your PCMH Clinic actively supports you in learning to manage and organize your own care at the level you choose. Recognizing that patients and families are core members of the care team, PCMH Clinics ensure that they are fully informed partners in establishing care plans.
