The Comprehensive and Supports Waivers are often referred to as the DD (Developmental Disability) Waivers. The Supports Waiver helps people of all ages who have an intellectual disability or ages 21-64 with an acquired brain injury. The Comprehensive Waiver also provides services to qualifying people of all ages who have an intellectual disability, or an acquired brain injury; however, individuals must meet additional emergency criteria to qualify.

Portal Access & Application Request
- Request access to the Wyoming Health Provider (WHP) portal. Please allow up to 48 hours for approval of your access request.
- Set up your WHP account & application correctly using the guides linked near the bottom of the Become a Service Provider page.
- If interested in case management, get pre-authorized before applying. See the Case Manager Applicants tab for details.
Preparation & Training
- Familiarize yourself with the HCBS website and DD Waivers Service Index & Rules.
- Download and review the Initial DD Certification Required Documents checklist.
- Complete all training modules listed in the Provider Training tab on this page and complete a Training Summary form for each module. Case manager applicants must also complete modules found in the Case Manager Training tab.
- Complete and maintain current CPR/First Aid certifications, if applicable. Certifications must include a hands-on component from a nationally recognized curriculum.
- If assisting participants with medications, maintain a current certificate of completion for the Division’s Medication Assistance Training (MAT).
Background Screenings
- Complete and successfully pass each portion of the multi-step DD Background Screenings.
Application Submittal
- Complete and submit your application using the link in the Wyoming Health Provider Portal (WHP) portal.
- Be sure to rename ALL files with DD WHP File Naming Conventions before uploading.
- Applications that have not been submitted after 90 days will be canceled.
WY Medicaid Provider Enrollment & Agreement
- Complete your Wyoming Medicaid Enrollment and sign a Provider Agreement once notified to do so.
- You must select the correct PRESM Provider Type during enrollment.
- Your Wyoming Medicaid Enrollment and Provider Agreement are separate from the HCBS provider certification and must be renewed every 5 years. Visit Medicaid Provider Enrollment & Billing for additional information.

AUTHORIZED TO APPLY Case manager applicants are pre-screened to ensure all minimum qualifications listed below are met. You must submit your official college transcripts and a current resume by listing yourself as Staff on the Demographics page of the WHP portal. You will then be prompted to upload your transcripts and resume for review.

Education & Experience
- Case managers must be employed or contracted by a qualified case management agency and must have:
- A Master’s degree from an accredited college or university in human services, social services, or a related field of study;
- A Bachelor’s degree from an accredited college or university in human services, social services, or a related field of study and one (1) year of related work experience in human or social services; or
- An Associate’s degree from an accredited college or university in human services, social services, or a related field of study and four (4) years of related work experience in human or social services.
- Related Fields of Study include: Counseling, Education, Gerontology, Nursing, Psychology, Rehabilitation, Sociology or a related degree, as approved by the Division.
Qualifications
- Case Managers must obtain an NPI number in their name and submit a Medicaid Provider Enrollment application to the Division.
- Maintain current CPR and First Aid Certification.
- Successfully pass and submit all required background screenings.
- Meet educational and work experience requirements as specified by the Division.
- In addition to Initial Provider Trainings, case managers must also complete the Initial Case Manager Training modules.
- Eight (8) hours of annual training in areas specified by the Division is also required. Certificates or confirmation of attendance must be available for Division review.

Individuals who complete all of the following training modules will meet training requirements specific to Chapter 45, Section 15 of Wyoming Medicaid Rules. Provider organizations may choose to develop their own training modules if their training covers the key elements of each topic specified in the Division module. If Division modules are used to meet training requirements, an HCBS Training Summary Form must be completed for each module.

Medication Assistance Training (MAT)
Chapter 45 of Wyoming Medicaid Rule, which governs the Comprehensive and Supports Waivers (DD Waivers), requires that any person providing medication assistance during waiver services must be certified in Medication Assistance Training (MAT) offered through the Division. This training is intended to provide the information necessary to safely support participants with their medication.
Before You Begin
- This training replaces the train-the-trainer format that was previously available. As current certifications expire, every medication assistant must complete this training to be certified.
- MAT is comprised of 9 training modules.
- You must complete each module and score 80% or higher on the quiz at the end of each module. Once all 9 modules are complete, you will receive a Medication Assistance Training Certificate.
- If you are working in another tab of the browser while you are viewing the training (multi-tasking), this can cause functionality issues with the training – up to and including timing issues and disabled features. Please be sure you are giving your full attention to the training.
- MAT is not currently designed for use on mobile devices, and some functionality issues may occur if you attempt to view the training on tablets or phones. Added functionality for mobile devices will be available in the coming months.
- We are currently working on updates to the presentation, so you will notice a difference in how some of the modules look. Please excuse us during our construction.
- Closed captions are not currently included, although this is a priority in the coming months. If the recorded module is not accessible for a student, they can contact Shirley Pratt at shirley.pratt@wyo.gov for alternate training materials.
