Provider Incentive Pilot Project

All applicants will be pre-screened to ensure they meet the folloiwng minimum qualifications: 

 1.  Completed a full certification renewal with over one (1) year of successful service provision in Wyoming

2.  Must be in good standing with no open corrective or adverse action

3.  Most recent certification renewal resulted in a 3-year recertification period

4.  Completed attestation regarding capacity to continue to serve current participants throughougt the project

5.  Consistently reported critical and non-critical incidents as defined by the Division with  demonstrated appropriate response to incidents and prompt action to comply with Division technical assistance

6. No corrective action in the past two (2) years

In addition to the criteria for for ALL applicants, case managers must also meet the following qualificiations:

1. Consistent with Division to successfully resolve Quality Improvement Review (QIR) findings

2. Completed UW Case Management Training during UW evaluation and support period

3. Demonstrated ability to write robust and supportive Positive Behavior Support Plan (PBSP)
