The HCBS Section manages Medicaid Waiver programs that provide individualized services and support at home and in the community for people with intellectual disabilities and other limitations. Each HCBS waiver program has specific eligibility requirements.

Become a Service Provider

HCBS Medicaid Service Provider Application Overview

The steps below offer a general overview, but specific requirements vary by waiver program. Visit the CCW or DD Initial Certification pages linked below for more details.

 1.  Portal Access & Application Request

Request Wyoming Health Provider (WHP) portal access. Allow up to 48 hours for approval. It is recommended that all training and background screenings be completed before requesting an application. 

2.  Preparation & Training

 Review rules and requirements, prepare documents, and complete all required training.

3.  Background Screenings

 Complete and successfully pass each portion of the multi-step background screening.

4.  Application Submittal

Applications are canceled if not submitted within 90 days. Rename all files before uploading to submit a complete application.  

5.  Medicaid Provider Enrollment

Enroll as a Medicaid Provider & sign the Wyoming Medicaid Provider Agreement.

 

Case Manager Applicants

Case Manager applicants are pre-screened and authorized to apply to ensure that minimum qualifications are met. Official transcripts and a current resume are required for pre-screening. 

Education & Experience

–  A Master’s degree from an accredited college or university in a related field of study;

 – A Bachelor’s degree in a related field of study and one (1) year of related work experience; or

– An Associate’s degree in a related field of study and four (4) years of related work experience.

Additional Training

In addition to completing the required provider trainings, case managers must also complete additional training prior to certification and on an annual basis.

Before You Apply

– Understand what services are available and which you plan to provide. To learn more, review the Waiver Service Indexes.

– Become familiar with the program’s Administrative Rules and HCBS requirements.

– Understand that all HCBS providers are Wyoming Medicaid Enrolled Providers; to provide HCBS Waiver Services, you must also enroll as a Wyoming Medicaid Provider and sign a provider agreement every 5 years.

– Keep in mind that applications not submitted within 90 days will be canceled & information will be lost.  Applicants must request a new application if this occurs.
