NOTICE OF PUBLIC FEEDBACK REQUEST

Community Choices Waiver Program

The Wyoming Department of Health, Division of Healthcare Financing (the Division), is preparing to submit a waiver renewal agreement for the Community Choices Waiver (CCW) program to the United States Department of Health and Human Services, Centers for Medicare and Medicaid Services (CMS). HCBS waivers are approved by CMS for 5 year periods. This renewal will have an effective date of July 1, 2026, following the expiration of the current waiver on June 30, 2025.

We invite your feedback on the draft of Appendix C of the renewal agreement. Appendix C details service definitions, provider qualifications, and performance measures for quality improvement. You can access the draft of Appendix C and a summary of proposed changes at the bottom of this notice.

Please submit written feedback to Adrienne Rosenberg at adrienne.rosenberg@wyo.gov by November 1, 2025. Kindly include the specific section, service, or page number to which your feedback pertains.

Additionally, we will host a Zoom meeting on October 16, 2025, from 1:00 PM to 2:00 PM, to receive verbal feedback on the Appendix C draft. Meeting details are as follows:
