CLIA transitioned to a paperless system. CMS longer mails CLIA certificates or paper fee coupons. Additionally, all CLIA certification and survey fees must now be paid online (payments by paper check are no longer accepted). Pay your CLIA fees online by clicking here
Laboratories must have a valid email address on file to receive electronic CLIA certificates, fee coupons, and other CMS communications.

- Reach out to wdh-ohls@wyo.gov to go paperless. Include your laboratory name, laboratory director or owner’s name, CLIA number, director or designee’s signature.

Complete a CLIA Application, in its entirety, for any of the following, and mail to: Healthcare Licensing and Surveys, Hathaway Building, Suite 510, 2300 Capitol Avenue, Cheyenne, WY 82002
− Initial Laboratory Certification
− Change in certification type
− Change in provider name
− Change in address, telephone number, etc.
− Change in laboratory director
