IMPORTANT NOTICE – PLEASE READ
If your laboratory receives a CLIA fee with a pink slip, this fee needs to be paid within two weeks of the date of the notice to avoid termination of your CLIA certificate. If your CLIA certificate terminates, you are no longer allowed to test or get reimbursed for testing. CLIA fees may be paid on www.pay.gov to ensure timely processing.
If your laboratory has closed, please provide written notification to the State Agency in the state the laboratory is located. Written notification must include laboratory name, CLIA number, name of the Laboratory Director and/or Owner, and the signature of the Laboratory Director.

− Initial Laboratory Certification
− Change in certification type
− Change in provider name
− Change in address, telephone number, etc.
− Change in laboratory director
