The Community Choice Waiver (CCW) assists older Wyoming adults (65+) who are on – or eligible for – Medicaid. It also aids individuals ages 19-64 with a confirmed disability that meets the Social Security Administration’s criteria.

Portal Access & Application Request
- Request access to the Wyoming Health Provider (WHP) portal. Please allow up to 48 hours for approval of your access request.
- Set up your WHP account & application correctly using the guides linked near the bottom of the Become a Service Provider page.

Preparation & Training
- Familiarize yourself with the HCBS website and CCW Waiver Service Index & Rules.
- Complete all Provider Agency Training and if applicable, Case Management Agency and Case Manager Training modules found in the tabs on this page.
- Complete a Training Summary form for each module.
- Download the CCW Provider Manual and if applicable, the CCW Case Manager Manual to become familiar with program expectations and requirements.
Background Screenings
- Complete and successfully pass each portion of the multi-step CCW Background Screenings.
Application Submittal
- Complete and submit your application using the link in the Wyoming Health Provider Portal (WHP) portal.
- Be sure to rename ALL files using the CCW WHP File Naming Conventions before uploading.
- Applications that have not been submitted after 90 days will be canceled.
WY Medicaid Provider Enrollment & Agreement
- Complete your Wyoming Medicaid Enrollment and sign a Provider Agreement once notified to do so.
- You must select the correct PRESM Provider Type during enrollment.
- Your Wyoming Medicaid Enrollment and Provider Agreement are separate from the HCBS provider certification and must be renewed every 5 years. Visit Medicaid Provider Enrollment & Billing for additional information.

Education & Experience
- Case managers must be employed or contracted by a qualified case management agency and must have:
- A Master’s degree from an accredited college or university in human services, social services, or a related field of study;
- A Bachelor’s degree from an accredited college or university in human services, social services, or a related field of study and one (1) year of related work experience in human or social services; or
- An Associate’s degree from an accredited college or university in human services, social services, or a related field of study and four (4) years of related work experience in human or social services.
Qualifications
- Case Managers must obtain an NPI number in their name and submit a Medicaid Provider Enrollment application to the Division.
- Successfully pass and submit all required background screenings.
- Meet educational and work experience requirements as specified by the Division.
- In addition to Initial Provider Trainings, case managers must also complete additional Case Manager Training Modules. Summaries of these trainings must be available for Division review.
- Annual review of the Division’s Case Manager Training Modules is also required. Summaries of these trainings must be available for review upon request.
