- Have an intellectual or developmental disability or related condition and
- Currently be receiving services on the Supports Waiver or Comprehensive Waiver or
- Currently be on the waiting list for the Supports Waiver or
- Currently in an institution (Wyoming State Hospital, nursing home, etc.) but eligible for waiver services upon discharge.
An applicant may also qualify if the person has been involved in recent critical incidents or events in the community. If appropriate supports and services are in place in the community and, when applicable, additional supports or consultations have been provided, but the applicant continues to experience significant challenges with aggression and violent behavior, wandering, sexually inappropriate behavior, self-endangering behaviors, or medication noncompliance, the Behavioral Health Department (BHD) Care Team will work with the applicant to assess the current situation.
An Interdisciplinary Team will determine if WLRC is the least restrictive, most appropriate and most integrated placement for the individual.
