Vital Statistics Services files corrections for persons whose vital event (birth, death, marriage or divorce) occurred in Wyoming. Filing a Court Ordered Name Change
- Obtain a certified copy of the court order that stipulates the name change from the court that issued the change and specifically states in the order that Vital Statistics Services is to amend or issue a new birth certificate.
- Complete the Application For Correction Form.
- Send the certified copy of the court order, the application, a photocopy of your current ID (such as a driver’s license) and a $55 fee to our office. The fee can be made via a personalized check (third party checks are not accepted) or money order made payable to Vital Statistics Services. The fee also includes one certified copy of the record. We must keep the certified copy of the order, so be sure to make a copy for yourself.

- Once the court order has been processed, we will not be able to make any additional changes to the certificate without another court order. It is very important the court order submitted is correct.

Adding a Father to the Birth Certificate without using a Court Order

Adding a Father to the Birth Certificate with a Court Order Establishing Paternity

Correcting a Birth Certificate for a child born in Wyoming
- After the child’s first birthday, our office will require documentary evidence and additional fees will apply.
- If you are seeking to change the child’s last name that correction can only be done through a court order submitted to our office or an affidavit acknowledging paternity (when applicable, please see those sections for more information).
- Complete the Application For Correction Form. A list of evidence that may be used to make this correction is on the back of this form.
- Send the evidence, the application, a photocopy of your current ID (such as a driver’s license) and fee to our office. If you have certificates to be replaced, please return these at this time also. The fee can be made via a personalized check (third party checks are not accepted) or money order made payable to Vital Statistics Services.

- It is highly recommended that the parent(s) submit a request for the newborn certificate to review for accuracy and address any errors as soon as possible. If the error is caught within the first year of the child’s birth, in most cases, Vital Statistics Services can make the correction free of charge. Correcting a Death Certificate for a Wyoming Record
- After a year, our office will require documentary evidence.
- If you are seeking to correct the medical information on the death record, this information can only be changed by the Medical Certifier.
- For all other corrections, complete the Application For Correction Form. A list of evidence that may be used to make this correction is on the back of this form.
- Send the evidence, the application, a photocopy of your current ID (such as a driver’s license) and fee to our office. If you have certificates to be replaced, please return these at this time also. Correcting a Divorce Certificate for a Wyoming Record
- Corrections must be initiated by the Court that filed the Divorce Decree.
- Once the court has made this correction, please complete the Application For Correction Form.
- Send the Nunc Pro Tunc from the Court, the application, a photocopy of your current ID (such as a driver’s license) and fee (if applicable) to our office.
