To obtain TB Treatment records contact the medical provider who prescribed the TB treatment .
If the medical provider who prescribed the TB treatment is no longer in practice and you received treatment through WDH TB Program Patient Assistance Program complete the records request form according to the directions and in the Information Released FROM section check Public Health Division and in the Information to be Released section write TB treatment records. Email or fax the completed form and a clear copy of your proof of identity to cdu.treatment@wyo.gov or 307-777-5279. For questions contact the TB Controller, Sarah Hendricks, at 307-777-6563.
