Eligibility
To be eligible for the Community Choices Waiver (CCW), you need to meet certain requirements on a continuous basis related to where you live, the level of care you need, and your finances. Read through the CCW Program Fact Sheet to learn more. To apply for the waiver, you must be eligible for Medicaid. If you’re not already on Medicaid, you can apply for it or renew your financial eligibility by clicking the button below.
Application Process
1. Review the Participant Handbook for eligibility and level of care assessment information:

2. Contact the Benefits and Eligibility Specialist in your area.
3. Select a Case Management Agency.
4. Complete the Community Choices Waiver Application.

5. A Public Health Nurse (PHN) will contact you to schedule your required LT101 assessment.
Apply for DD Waivers
Eligibility
To be part of the Supports and Comprehensive Waiver programs, you need to be eligible to receive Medicaid. If you’re not already on Medicaid, you can apply for it or renew your financial eligibility for ongoing waiver services by clicking the button below:

Application Process
1. Review the Application Guide for the Supports Waiver, which lists eligibility requirements.

2. Contact your area Benefits and Eligibility Specialist.
3. Select a Case Manager and complete the Case Management Selection Form.

4. Work with the selected Case Manager to plan the best services for your individualized plan of care.

Under circumstances outlined in 2017 House Enrolled Act 0600, qualifying dependents of active military service members may apply for waiver services before residing in the state of Wyoming or, upon their return to the state of Wyoming, resume waitlist placement or funding status identical to where they would be had they not left the state for military reasons. If you have questions about the rights and responsibilities of qualified dependents of military service members, please contact Jenifer Adams at jennifer.adams@wyo.gov.
