All amendments must be done through the mail, forms and payments may not be sent electronically.
Court Ordered Amendments
The following is required for court ordered amendments:
- Certified Copy of the Court Order
- A photocopy of your current ID
- Application for Correction Form
- $55 fee (We do not accept third party checks)
Please note, some court orders may require additional evidence.
Corrections In the First Year
The following is required for changes within one year:
- Application for Correction Form
- A photocopy of your current ID
- Fees may apply for corrections
Corrections After the First Year
The following is required for changes after the first year:
- Application for Correction Form
- A photocopy of your current ID
- Evidence (Suggested documents: Medical Record, Military Record, Insurance Policy, School Record, Notarized statements from an officiant, etc.)
- $20 fee for correction only
- $25 fee if requesting a new certificate also
