- Preferred Placement Location (On-site, Remote, Hybrid)
- Desired PHD Unit(s) to Partner With
- The duration requirement for this internship (months, hours, time frame, etc.).
- Description of your ideal internship project.
- Why are you interested in WDH for your internship?
Once submitted, you may be requested to answer any remaining questions pertaining to your placement within the Wyoming Department of Health’s Public Health Division.
