Eligible Applicants: Wyoming organizations that are in good standing with the Wyoming Secretary of State and registered on SAM.gov. Any funded applicant must have a fiscal agent through which grant funds can be allocated. An awardee may serve as their own fiscal agent. Preference may be given to organizations with strong relationships with health systems.
Description: This funding opportunity will assist Wyoming organizations with the implementation and evaluation of evidence-based strategies to prevent and manage cardiovascular disease (CVD) and diabetes in high burden communities within Wyoming. The chosen applicants will enable the CDPP to accomplish grant objectives and promote the implementation of Diabetes Self-Management Education and Support (DSMES), National Diabetes Prevention Program (National DPP), and Self-Measured Blood Pressure Monitoring (SMBP) throughout Wyoming.
