Eligible Applicants: Wyoming organizations that are in good standing with the Wyoming Secretary of State and registered on SAM.gov. Any funded applicant must have a fiscal agent through which grant funds can be allocated. Awardee may serve as their own fiscal agent.
Description: This funding opportunity will support Wyoming organizations in implementing and evaluating evidence-based strategies to prevent and manage cardiovascular disease (CVD) in high-burden communities. Organizations who are awarded the grant will be required to plan and implement an evidence-based chronic disease management program for high blood pressure and high cholesterol. Awarded applicants will be required to dedicate 535 hours to the grant.
