Wyoming Statute and the Wyoming Rules and Regulations for School Immunizations state that a report of the immunization status and conditional enrollment of pupils in every School and Child Caring Facility shall be sent once a year to the Wyoming Department of Health no later than November 1 of each year.
The 2016 Immunization Assessment is due November 1. If you have not yet completed or were granted an extension, please complete the assessment as soon as possible. Immunization Unit staff will be following up with schools and facilities that have not yet submitted.
