Schools and Child Caring Facilities are valuable partners in the fight against vaccine-preventable diseases. The Immunization Program is committed to providing you with the resources, information, and tools needed to ensure children are immunized appropriately. We thank you for your hard work and dedication to protecting the children under your care.
Rules and Regulations
Parents must submit proof of immunization to schools, preschools, and child caring facilities upon entry for all children six (6) months or age and older. School and child caring facility administrators shall not retain a child in their facility for more than thirty (30) calendar days without proof of immunization that is in accordance with the schedule published by the State Health Officer or an approved exemption to the mandatory immunizations.
In the event of a vaccine-preventable disease outbreak, all children who are not fully immunized against the specified disease, or those granted an exemption for the vaccine that prevents the specified disease, shall be excluded from attending the school or child caring facility during the existence of the outbreak.
WDH Rules and Regulations for Mandatory Immunizations
The Department of Health has promulgated the following rules and regulations for the management and control of communicable disease to comply with W.S. § 35-1-240(a)(ii), W.S. § 35-4-101, W.S. § 21-4-309, and W.S. § 14-4-116, and the Wyoming Administrative Procedure Act of W.S. § 16-3-101, et seq.

Health Insurance Portability and Accountability Act (HIPAA) Omnibus Rule – For Schools
Blank Official Record of Immunization: This form can be provided to parents or guardians and is to be completed by a licensed Physician/Designee or Public Health Authority. Once complete, it can be accepted as proof of immunization.

Note: If your district’s registration occurs prior to identifying the specific school a child will attend, it is acceptable to write the school district name on the agreement, rather than the specific school name. However, the form must be included in the student’s record at the school in which they will attend, when identified, and be made available to WDH staff upon request.
In addition to the Agreement provided by WDH. The following forms of documented consent are also permitted and if used, must be made available to WDH upon request:
- Dated note in the student record documenting verbal permission
- Consent captured in a student management system (language must be consistent with the Agreement provided by WDH).
