Parents, guardians, or emancipated students may apply for an exemption to the mandatory immunizations required for entry into schools and child caring facilities. Wyoming currently offers two exemptions types, Medical and Religious. Wyoming does not have a philosophical or personal belief exemption.
The Medical Exemption to Mandatory School Immunizations form may be completed if a child has a medical contraindication to the administration of any vaccine. A physician’s statement can also be accepted.
A Religious Exemption to Mandatory School Immunizations form may be completed if there is a religious objection to the administration of any vaccine.
To apply for an exemption, complete the most appropriate form below. The form must be completed in its entirety and must contain original signatures; photocopies and faxes are not accepted. Exemption requests may be approved by either the State Health Officer or your local County Health Officer.
Exemption requests should be submitted in their original completed form to either your local county Public Health Nursing Office for approval by the County Health Officer, or to the State Health Officer at: Wyoming Department of Health, 6101 Yellowstone Road, Suite 420, Cheyenne, WY 82002, Attn: Immunization Exemptions.
