About the PATH grant
SAMHSA’s Projects for Assistance in Transition from Homelessness (PATH) funds services for people with serious mental illness (SMI) experiencing homelessness.
For FFY2018 and 19, Wyoming contracts with five community providers to assist people who are experiencing homelessness and have a recent diagnosis of SMI. These agencies can provide coordination services and other resources:
Sweetwater Family Resource Center
Community Action Partnership–Natrona County
Eastern Shoshone Recovery Program (307) 335-1169 (Wind River Indian Reservation)
Recover Wyoming (Cheyenne)
YES House (Gillette: for youth aged 18 – 25 only)
