Registry (HLS)
Access: Healthcare Licensing and Surveys maintains a registry of all nurse aides who are certified to provide services in nursing care facilities and swing beds licensed by the Wyoming Department of Health. If a CNA is not listed on the registry, that individual is not eligible to work as a CNA in a nursing care facility or swing bed in Wyoming

Placement. For placement on the HLS Certified Nurse Aide Registry:
- Fill out the following HLS/CNA-109 form and fax or mail it to Healthcare Licensing and Surveys (address and fax number are provided on the form).

- Please note: Graduate Nursing Assistants (GNAs) cannot be placed on the Healthcare Licensing and Surveys Certified Nurse Aide Registry.
