Renewing is easy. It can be completed online, over the phone or by fax.
Children are enrolled to the program for 12 months. Two months before your child’s coverage end date, you will receive a renewal form in the mail. Please complete the form and return to us by mail or fax. You may also call us and complete the renewal process over the phone.
Automated renewal reminder calls will occur at one month and two weeks prior to your child’s coverage ending.
*If you are unable to complete the renewal process prior to your child’s coverage end date, you will need to reapply for Kid Care CHIP.
