PROVIDER ENROLLMENT
To make the DD Children's Waiver run we need organizations, individuals and companies to provide various services for the special needs children. The various services were explained in a previous section of the manual. Anyone who wishes to be a provider can as long as they meet the eligibility criteria for that specific service.
Potential providers make their application through the Area Resource Specialist (ARS), Division of Developmental Disabilities-Department of Health. All waiver services and their standards are identified in the waiver document. The provider is responsible to understand all the standards for the certified service. The following is a list of services and examples of the standards needed to become certified:

Special medical equipment & supplies - companies throughout the state and other states can enroll, must meet appropriate licensure laws;
- Environmental modifications - if the city/town, which the modifications are to be done, requires a contractor's license then a copy of that license is required;
- Skilled Nursing - a copy of the individual's license is required, must be a Registered Nurse;
- Respiratory Therapy - a copy of their certification with the National Board of Respiratory Care;
- Psychological - a copy of their license;
- Dietitian - a copy of their license by the Commission of Dietetic Registration;
- Respite - we require DOB verification, FBI & State background check, Central Registry Screen - provider & adult family members, three references, alias form, general medical exam, liability insurance (if possible), interview by the ARS, and scout certification letter that verifies the standards have been met. Respite providers will be limited to caring for no more that two (waiver or non-waiver) children at one time;
- Homemaker - we require DOB verification, bonding insurance , and an interview by the ARS that says the individual is skilled in performing homemaking tasks, is knowledgeable in and has the ability to respond to household emergencies;
- Personal Care - we require DOB verification, FBI & State background check, three references, liability insurance (if possible), alias form, general medical exam, Central Registry Screen - provider & adult family members, interview by the ARS and scout certification letter that verifies the standards have been met;
- Residential Habilitation Trainer - the individual must have two (2) years of college and two (2) years of mental retardation work experience;
- Special Family Habilitation Home - we require FBI & DCI background checks - provider & adult family members, Central Registry Screen - provider & adult family members, DOB verification, three references, alias form, general medical exam, home assessment, liability insurance (if possible),training, child and parent/guardian visit, interview by the ARS and the scout certification letter that verifies the standards have been met. Special Family Habilitation Home providers can care for no more than two individuals (waiver or non-waiver) outside the Special Family Habilitation Home provider's natural family.
- Individualized Support Service Coordinator - the individual must have a Bachelor of Arts degree or be supervised by a regional service provider.
Each provider must complete the Consultec enrollment package, which consists of three forms, and the Payee Information form from the Auditor's office so they can get paid.
The Division of Developmental Disabilities certifies the provider in the area of service applied for. The certification by the DD Division is based on the assurance to Health Care Financing, by the DD Division, that the necessary safeguards have been taken to protect the health and welfare of recipients of waiver services. The Federal guidance on this matter is that the standards are in place before certification is granted by the DD Division. Substantial compliance is required before a prospective applicant is approved as an eligible Medicaid approved provider. Substantial compliance means that there is a positive track record of actual service provision along with above documentation.
