Disease Prevention and Health Promotion

This program encourages senior centers to develop special preventive health programs. If you would like to see what Prevention and Promotion services are in which county, please view the OAA services provider index.

Health risk assessments - Assessments done by a individual to evaluate the health status of the older adult.

Routine health screening - Screenings for certain diseases or conditions which may include hypertension, glaucoma, cholesterol, cancer, vision, hearing, diabetes, and nutrition.

Fitness activities - Organized activities that promote healthy older adults. These activities incorporate cardiovascular, muscle toning and agility improvement.

Nutrition counseling - Provision of individualized advice and guidance to individuals, who are at nutritional risk because of their health or nutritional history, dietary intake, medications use, or chronic illness. Guidance on options and methods for improving their nutritional status. Services are performed by a health professional in accordance with state law and policy.

Education for individual or groups - Programs to promote better health (physical or mental) by providing accurate health information and instruction to participants and/or care givers in a group or individual setting overseen by an individual with health experience.

Health promotion programs - Programs relating to chronic disabling conditions( including osteoporosis and cardiovascular disease), alcohol, and substance abuse reduction, smoking cessation, weight loss and control, and stress management.

Home injury control services - Screening of high-risk home environments and provision of educational programs on injury prevention (including fall and fracture prevention) in the home environment.

Medication management - Oversight of medications by a RN for those older adults who have been assessed to require management of their medications.

Informational programs concerning Medicare benefits - Educational programs on availability, benefits and appropriate use of preventive health services covered under Medicare.

Eligibility for these services: A client must be over 60 years of age. Additional eligibility for nutrtional counseling include: a spouse of an individual over 60 years of age, or a disabled person living in senior housing attached to a congregate meals site.
